Select the entity type that matches your business structure. If you work as an individual without a registered business, select "Individual / Sole Proprietor". For LLCs, Corporations, or Partnerships, select the appropriate option and use your legal business name.
Your W-9 information can be found on your most recent tax return or in your business formation documents. You'll need your legal name (or business name), address, and Tax Identification Number (TIN/EIN or SSN).
A notice address is where official legal correspondence will be sent. This may differ from your primary contact address. If you want notices sent to the same address as your contact info, you can indicate "Same as contact" on the MSA.
Direct deposit is required for all H&J consultants. It's faster, safer, and more convenient than paper checks. You'll need your bank routing number and account number. This information is encrypted and stored securely.
The DWC-85 is a Texas workers' compensation form. It confirms you understand you're an independent contractor, not an employee, and that H&J's workers' comp insurance does NOT cover you. Texas law requires it (Texas Labor Code §406.122).
